Student Health Services
It is the policy of Butler University that all full-time students will meet the following health requirements by the indicated due dates based upon fall (August 1) or spring (January 1) start date. Failure to meet the below health requirements by the due date will result in a HOLD being placed on the student’s account. This hold prevents the student’s ability to change, add, or drop any classes and register for the following semester.
2) Physical exam (completed within the 12 months prior to start date)
3) Mandatory online questionnaires within the student health portal: Health History and TB Risk Screen
4) Mandatory proof of health insurance: Know Your Options
Medical and/or religious exemption requests can be submitted in accordance with IC 20-34-4. In the event of an outbreak of a vaccine-preventable disease for which you (or your minor student) are not fully vaccinated, you or your student may be excluded from school to protect your/their health and the health of our students and staff. Students must sign and acknowledge the exemption request and comply with the consequences of incomplete vaccination status.

Policy Target Audience
All Butler University students that are considered full-time undergraduate or full-time graduate and attend courses on campus. This policy also applies to all online healthcare programs that have a clinical rotation component.
Definitions
Full-time undergraduate: student taking a minimum of 12 credits or more.
Full-time graduate: student taking a minimum of 6 credits or more.
